Before seeing Dr. Moses, please make sure to complete the registration forms required for your visit. If you had previous imaging, please make sure to bring both a CD of the images as well as the report. You may find more information about how to prepare for your appointment below.

  • Additionally, compile a list of all medications you're currently taking, including dosages, as well as any known allergies or adverse reactions to medications.

  • Don't forget to bring your insurance information, ID, and any necessary referral forms if required by your insurance provider.

  • Lastly, it can be beneficial to jot down questions or concerns you have about your condition or treatment options to ensure that you get the most out of your consultation. This proactive approach will help streamline the evaluation process and enable Dr. Moses to provide you with the most accurate and effective care.

      • Do you have a family history of similar symptoms or conditions?

      • Have you experienced any recent injuries or trauma to the affected area?

      • Are you currently taking any medications that might be related to your symptoms?

      • Do you have any underlying medical conditions or chronic illnesses that could be contributing to your symptoms?

      • Have you had any relevant diagnostic tests, such as X-rays, MRIs, or nerve conduction studies, and what were the results?

      • Have you seen other healthcare providers or specialists for these symptoms, and if so, what treatments or interventions have you tried?
